Community Therapy team - Elderly Medicine, LUHFT-wide
You are being referred to the Liverpool Stroke Recovery Partnership (LSRP) for your continued rehabilitation after leaving hospital.
The Team consists of specialised professionals from Liverpool University Hospitals NHS Foundation Trust and the Stroke Association.
What is the Liverpool Stroke Recovery Partnership (LSRP)? The LSRP is the multidisciplinary team that will manage your ongoing rehabilitation needs when you are discharged from hospital.
This service can last for up to six months as required and the number of visits will depend on your individual needs.
Your therapy programme will involve one or more of the following:
• Sessions in your home (or residential care home).
• Sessions at local therapy hubs or hospital.
• Group activities – Walk to health, Minds in motion.
• Online groups (Neurorehabilitation Online – NROL).
What are the benefits of the LSRP?
• You can leave the hospital earlier i.e. be discharged from the ward once you are medically fit.
• You can get back to your normal family/social/work activities more quickly.
• You can have personalised therapy sessions and plans in your own environment. The aims of the LSRP
• We offer an active rehabilitation programme which will help you achieve your goals, and which promotes your independence in normal daily activities.
• We can give advice, support and education to patients, families and carers.
What do we expect from you?
• We expect your active participation in your goal setting and rehabilitation programme. This will include practicing your exercises and activities between your treatment sessions.
• Flexibility with treatment times – You will be given an appointment date and time, but therapists may arrive within an hour either side of this.
• Please ensure you contact the team beforehand if you are unable to keep an appointment. Please note if more than three appointments are missed without informing the team prior then you may be discharged from this service.
• If you notice any changes in your health or in your ability to do any tasks, please inform the therapist at each session.
• The Trust’s operate a no smoking policy, and we ask that you do not smoke two hours before or during a treatment session.
• The Trust’s operate a Zero Tolerance Policy to violence and aggression against staff.
• You may also have to remove pets or ensure they are in another room whilst therapist present.
What will happen if I decide this is not for me?
You will be given the opportunity to discuss alternative options with members of the Stroke Team before you are discharged. If you have any concerns, please speak to a member of the Stroke team. Therapists aim to visit you within 48 hours after you are discharged.
You will be contacted from 8 am onwards for an appointment to be arranged via phone call- this may be from a private number or a mobile phone.

Your feedback is important to us and helps us influence care in the future. Following your discharge from hospital or attendance at your outpatient appointment you will receive a text asking if you would recommend our service to others. Please take the time to text back, you will not be charged for the text and can opt out at any point. Your co operation is greatly appreciated.
