Register for My Pregnancy Notes
Congratulations on your pregnancy, it's now time to sign up to My Pregnancy Notes.
Please register your pregnancy with us as soon as you can as this will help us to support you early in your pregnancy so that your appointments/ scans are completed on time.
The process of registering for My Pregnancy Notes informs us that you are pregnant and this is the quickest way of registering your pregnancy with us. There is no need to call us to make an appointment.
Once registered, our access centre team will match your application to your record within five working days and book your first appointment with a midwife. This appointment will be visible by clicking ‘Clinical Record’ in the top right, ‘Care Details’ tab, and then ‘Appointments’. You will also receive a letter in the post.
However, if you are not able to register on-line, you can call 0151 702 4328, Mondays to Fridays, 8am-6pm and a member of our team can assist you.
When there is not enough time to send a letter by post, we will sometimes call you from a an ‘unknown caller id’.
If you are not already taking them, we recommend that you take 400micrograms of folic acid every day until you are 12 weeks pregnant. You should also take 10 micrograms of vitamin D every day throughout your pregnancy. You can buy these from a pharmacy or supermarket or ask your GP to prescribe them. You may be able to get free vitamins though the Healthy Start Programme. Please ask your midwife for more information.

- Click Register
- Click I am a pregnant woman (even if you are a health professional who is currently pregnant)
- Complete registration details
- You must opt to share your information with us, otherwise we cannot give you access
- Your email will become your username
- Click 'Verify Email'
- Navigate to your email
- Open email
- Click 'Verify Email'.
By signing up to My Pregnancy Notes, you will be able to engage with your care like never before, access your pregnancy record on the go on any device, and share your record with other healthcare professionals.
My pregnancy notes gives you access to:
- Your appointment dates/times, other than scan appointments. These will be visible very soon.
- Your personalised management plans and consultations
- Local and national leaflets and videos that support your decision making about your care
- Outcomes of referrals made
- Maternity contact numbers
- Your antenatal, birth, and postnatal plans (My Preferences)
- Blood results. These are manually input by your midwife and may not be updated until your next appointment, but you will be contacted prior if any results are abnormal.
- Your booking notes, labour, delivery and discharge summaries
- What can you not access?
- Scan reports and images
- Your Liverpool Women’s growth chart. You will be given a separate login for this following your dating scan.
- Inpatient notes.
Once registered, you can inform us of any future pregnancies.
You can do this by:
- Logging in
- Click 'My Account' and 'New Pregnancy'.
Our access team will create you a new pregnancy record, link your application to it, and make you a booking in appointment within five working days.
If you would like to inform us of a pregnancy loss, termination of pregnancy, or if you transfer your care to another Trust, please visit the 'My Notes' tab and 'Notify My Hospital'.
This will let our access team know to cancel all of your future appointments without the need to call us.
You can also share your record with your local midwife or another hospital if you live outside of the Liverpool area by granting an access token.
Follow these simple steps to add My Pregnancy Notes to your home screen:
Open your web browser and launch My Pregnancy Notes.

- Scroll down and select 'Add to home screen'
- Click 'Add'.

- Select this action button in the top right of the browser
- Scroll down and select 'Add to home screen'
- Click 'Add'.
