We are launching 'Hospital to Home' to help colleagues understand the aim of the Integrated Health and Social Care programme and communicate the importance behind getting patients home sooner.
‘Hospital to Home’ is what we should all be thinking about. From the minute a patient arrives in our hospitals whether they come in by ambulance, or by foot – how can we get our patient from Hospital to Home. We all have a role to play.
