What is Patient Pass
Patient Pass is a secure online centralised platform that facilitates the referral of patients between non-GP complex referral pathways, potentially requiring inter-hospital transfer. It allows for seamless sharing of clinical details, management plans, and tracking of patient progress throughout the transfer process. Patient Pass will aim to mitigate against existing clinical safety risks and provide a robust solution to address poor communication, incomplete joint treatment plans, transfer handovers, and referrals being sent directly to clinicians.
Key Features
- Real-time communication: Keeps referring clinicians and specialists informed instantly through secure two-way messaging.
- Audit trail: Provides a record of all interactions on the site.
- Automated workflow: Ensures timely prioritisation and triage of urgent cases, with automated workflows ensuring next-day triage where appropriate.
- Centralised referral hub: Replaces manual processes with a single, standardised digital solution.
- Improved operational intelligence: Improves patient flow and capacity management by providing visibility of bed demand and patients which can be safely managed remotely.
