What is Patient Pass 

Patient Pass is a secure online centralised platform that facilitates the referral of patients between non-GP complex referral pathways, potentially requiring inter-hospital transfer. It allows for seamless sharing of clinical details, management plans, and tracking of patient progress throughout the transfer process. Patient Pass will aim to mitigate against existing clinical safety risks and provide a robust solution to address poor communication, incomplete joint treatment plans, transfer handovers, and referrals being sent directly to clinicians.

How do I Access Patient Pass

Patient Pass can be accessed through patientpass.uhliverpool.nhs.uk via a web browser (Chrome, Firefox, Internet Explorer). However, all users will have to set up an account using their GMC number and authenticate before access is granted  to the platform. Please refer to the user guide for more information.

Yes. Patient Pass is designed to provide real-time status updates, message history and automated notifications when changes have been made to a referral. 

Please, contact the switchboard of the respective speciality you intend to speak with. However, ensure, follow-ups are made via Patient Pass, and the patient is added into the system. 

University Hospitals of Liverpool Group staff can access training via the Digital Faculty team. However, an assigned system champion (train-the-trainer) is available across the stated departments, and a comprehensive user guide can be found here to support you.

Patient Pass is tested for 99% availability. However, business continuity procedures have been put in place.
