What is Hospital to Home?
We want to keep our hospitals free for those who need our care and decrease the risk of patients developing further complications, linked to being in hospital longer than they need to and get them back to a place where they can recover or continue their care.
We know that once medically fit, our hospitals are not the most suitable place for our patients and keeping them within a hospital environment for longer than they need to be places them at risk.
This is why it is important that we make it a priority to get them home.
We are encouraging everyone to think about ways to get our patients from Hospital to Home sooner.
